Female, age 26, with itchy skin inflammation (atopic dermatitis), hyperactive immune system, asthma, very high immunoglobulin E (IgE) levels, and swollen lymph nodes (lymphadenopathy)

Description

As a young child, the patient had eczema, allergies, and asthma.

At age 19, her symptoms started to get much worse. She developed severe itchy skin inflammation (atopic dermatitis) on her face and entire body. This inflammation improved with cyclosporine, but came back when the medication dose was lowered.

The patient has been found to have very high IgE levels. She also has swollen lymph nodes (lymphadenopathy) that blocked circulation to her legs, a kidney (renal) cyst, arthritis, migraines, vertigo, and medically-induced depression.

Recently the patient was started on duplilmab, which has almost completely cured her symptoms.

Symptoms / Signs
  • Itchy skin inflammation (atopic dermatitis)
  • Full-body eczema
  • Hay fever (allergic rhinitis)
  • Swollen lymph nodes (lymphadenopathy)
  • Fatigue
  • Migraines
  • Vertigo
  • Hair loss
  • Scaly patches and red skin on scalp (seborrheic dermatitis)
  • Astigmatism
  • Material accumulating in outer layer of the eye (corneal dystrophy)
  • Difficulty seeing in dim light (nyctalopia)
  • Prominent corneal nerve fibers
  • Abnormality of the outer layer of the eye (corneal stroma)
  • Nearsightedness (myopia)
  • Delayed loss of baby teeth (persistence of primary teeth)
  • Abnormality of the jaw joint (temporomandibular joint)
  • Chest pain
  • Recurrent infections (sinopulmonary)
  • Recurrent bronchitis as a child
  • Asthma
  • Kidney (renal) and ovarian cysts
  • Enlarged liver (hepatomegaly)
  • Swollen legs (edema of lower limbs)
  • Very high IgE levels
  • Abnormal white blood cell levels (eosinophilia)
  • Heat intolerance
  • Gluten and food allergies
  • Abnormal temperature regulation

  • Prednisone- atopic dermatitis
  • Mycophenolate- atopic dermatitis, immune suppressant
  • Singulair- asthma
  • Amitriptyline- migraines, vertigo
  • Zytrec and Bendadryl- allergies, atopic dermatitis
